Everything you need to enrol a senior at our centre — the required documents and the steps, explained simply.

Enrolment at our centre is based on a short assessment and a file of documents, so that every senior receives exactly the care they need. Below you'll find the enrolment steps and the required documents, explained in plain language. And you're not alone: we help you, with our specialists and partners, at every step.

    Call or message us; we'll answer all your questions and arrange a meeting.

    You bring the required documents for the senior, complete.

    We review the file together and tell you if anything is missing.

    Social & medical assessment

    We assess the senior's social and medical needs to prepare the most suitable care.

    Admission visit or interview

    We meet in person and, where applicable, arrange a visit or a short admission interview.

    Admission approval

    We review the file and issue the admission decision.

    Signing the contract

    We sign the social services contract together, with every condition clearly explained.

    Setting the move-in date

    We set the move-in date and, if needed, you hand over the original documents.

    Welcome to the centre

    We welcome the senior and complete the admission paperwork. From here on, they're in our care.

Documents required for enrolment

Prepare the documents below. Depending on each person's situation, some apply only “where applicable” — our team will tell you exactly what you need.

Identity & civil status

  • Application for admission, completed by the senior or their legal representative
  • Copy of the senior's identity card
  • Copy of the birth certificate
  • Copy of the marriage certificate, divorce decree or death certificate of the spouse — where applicable
  • Copy of the identity card of the family member or legal representative

Income

  • Pension slip or pension decision (or other proof of income)
  • Documents on the senior's income and, where applicable, that of the legally liable support providers

Medical documents

  • A medical letter and a certificate from the family doctor or specialist confirming the person has no infectious or contagious diseases on record, listing all diagnoses
  • Recent medical tests — depending on the diagnosis: chest X-ray, ECG, psychiatric/psychological assessment, stool parasitology test, etc.

Social & legal documents

  • A comprehensive social inquiry (we help you, with our specialists — ideally issued by the town hall)
  • The court order establishing guardianship or conservatorship, together with the identity card of the guardian/conservator — if the person is under guardianship or conservatorship (we can help: we work with lawyers)

You don't have to manage this on your own. We work with lawyers, doctors and specialists and help you at every step — from the social inquiry and medical assessments to guardianship or conservatorship. Depending on the case, additional documents may be needed (photographs, GDPR consent, declarations from family members or further medical tests); we'll let you know in advance.

This is only the first step: a request, not an enrolment. It commits you to nothing. The actual enrolment happens after you talk to our staff, after the social and medical assessment and after the contract is signed.
